This is handwriting, not a diagram mark, and it is the note most often left out of a machine translation. The inspector looked under the car and found rust.
It is also the note that most changes a bid, because the underside is where the structure lives: rails, sills, subframe mounts, floor pans, fuel and brake lines.
The note has no severity scale. It means somebody saw rust from below and thought it worth writing down, which on a car graded 4 or better is a stronger signal than the same rust on a car graded 3.
What it should do to your bid
Get the car on a lift in Japan, or bid as though the number is high. This note is where imported cars turn into projects.
